Book a Free Digital AuditCreate a form
Go to Pharmacy → Forms → Create form. Add fields (text, choice, date, etc.) and publish when ready. Free plan includes one pharmacy form; Basic and Premium include unlimited forms.
Link products
Under Pharmacy → Linked products, assign a form to products or collections. Customers must complete the form before adding linked items to cart.
Review submissions
Pharmacy → Submissions lists completed questionnaires with PDF export. Use status and notes for your internal workflow — PharmaBoost does not make clinical decisions.
